The applicant for a Temporary Practice Permit must submit the following documents along with an application for the permit: 1. Request form for Temporary Practice Permit (REA 3009) form that can be submitted online. 2. Consent of Service of Process (REA 3006) form. 3. Copy of contract or letter of engagement with the physical address(s) or location(s) of the property or properties indicated. 4. Proof of the valid credential in the home state. 5.
