Which US nursing homes are in the CMS Special Focus Facility (SFF) programme, which are candidates for it, which have graduated and which have left Medicare and Medicaid altogether. Each record is one facility, identified by its six-digit CMS certification number (provider number), carrying the facility name, address, city, state, ZIP and phone exactly as CMS prints them, which of the four tables it appears in, the date of its most recent inspection, whether that inspection met the graduation criteria, and how many months it has been an SFF. Answers 'is this nursing home a special focus facility', 'which nursing homes in Ohio are on the SFF list', 'how long has this home been under enhanced oversight', 'which homes graduated from the SFF program', and 'which nursing homes were terminated from Medicare'. CMS publishes this as a single PDF that is overwritten every month, with no archive index, no per-facility page and no statement of what changed, so a month-stamped question — and people search month-stamped, see the autocomplete evidence in state/territories/nursing-home-special-focus-facilities.json — has no published answer at all.
